Families of our middle school athletes: It’s time to complete the fall sports participation packet!

To participate in fall sports, please make sure your athlete’s packet is completed and submitted to the middle school office.

You have several options:

+ Print, complete, and sign the packet, then scan and email it to Aaron Schultz at aschultz@wrvsu.org.

+ Drop off the completed packet at the middle school office.

+ Pick up a packet at school, complete it, and return it to the office.

Please be sure all required information and signatures are included so your athlete is ready to participate when the season begins!
